STEP 1:

Complete the Complimentary Lens Order Request Form below.

STEP 2:

Upon completion, your COL Sales Consultant will be immediately notified and will work with you to review lens options and treatments to produce the best possible outcome utilizing the latest lens technologies.

STEP 3:

You will be contacted by your Sales Consultant and provided a unique code. When placing your Complimentary Job Order, this code should be applied.
